How does it work?
Cardiolipin is an important phospholipid in the inner mitochondrial membrane and helps organise the respiratory complexes. SS-31/elamipretide is designed to protect or stabilise this environment, with the aim of better electron transport, less oxidative stress and better ATP production in disease models. Transferring findings from drug studies to a research product must be done with great caution.
Technical mechanism
Mitochondria-targeted tetrapeptide that binds cardiolipin in the inner mitochondrial membrane. Research investigates bioenergetics, oxidative stress and mitochondrial disease.
What is being researched?
- Primary mitochondrial myopathy
- Barth syndrome and cardiolipin-related mitochondrial disorders
- Age-related macular degeneration and geographic atrophy
- Preclinical models of ischaemia, oxidative stress and mitochondrial dysfunction
About the evidence: The research base for SS-31/elamipretide includes clinical studies. However, these concern specific pharmaceutical candidates and study designs, not general use of raw research material.

Mitochondrial mechanism
The foundational literature explains cardiolipin binding and mitochondrial bioenergetics.
- First-in-class cardiolipin-protective compound as a therapeutic agent to restore mitochondrial bioenergetics ReviewPubMed· 2014
This article describes elamipretide/SS-31 as a cardiolipin-protective compound and explains why mitochondrial bioenergetics is a central research target. It provides mechanistic background for the product group.
- Contemporary insights into elamipretide's mitochondrial mechanism of action and therapeutic effects ReviewPubMed· 2025
This recent review summarises the mechanism and therapeutic research areas for elamipretide. It is useful for an up-to-date, but still research-based, understanding of the substance.
Clinical research programmes
Elamipretide has been investigated in several controlled or open-label clinical programmes.
- Efficacy and Safety of Elamipretide in Individuals With Primary Mitochondrial Myopathy: The MMPOWER-3 Randomized Clinical Trial Clinical (human)PubMed· 2023
This randomised study investigated elamipretide in people with primary mitochondrial myopathy. It is relevant as clinical research data for the drug candidate, but cannot be transferred directly to generic research material.
- ReCLAIM-2: A Randomized Phase II Clinical Trial Evaluating Elamipretide in Age-related Macular Degeneration, Geographic Atrophy Growth, Visual Function, and Ellipsoid Zone Preservation Clinical (human)PubMed· 2025
This phase 2 study investigated elamipretide in dry age-related macular degeneration with geographic atrophy. It shows the breadth of clinical mitochondrial research, but is not documentation for product use outside controlled studies.
- Elamipretide: The first cardiolipin-directed mitochondrial therapeutic for Barth syndrome approved under accelerated approval ReviewPubMed· 2026
This article discusses elamipretide in the context of Barth syndrome and regulatory development. It provides important pharmaceutical context for SS-31/elamipretide, but does not apply to generic research products.
